Help & Care at Home When it's Most Needed...

RN's and LPN's:

Your clinical team provides skilled medical care for a variety of conditions:

  • Cardiac; CHF, COPD, telehealth
  • Diabetes management
  • Home infusion/IV therapies
  • Wound, ostomy and continence care
  • Hip and knee joint replacement
  • Oncology home care program

Home Health Aides

  • Provide personal care, including help with bathing and dressing
  • Light cleaning services for the individual under care
  • Assistance with food shopping and meal preparation

Medical Social Workers

  • Address family, social and emotional issues
  • Assist with access to appropriate community resources

Therapists

  • Physical Therapy:
    focuses on walking, balancing and using assistive devices (such as wheel chairs, walkers and canes).
    Our goal is to return our patient to their highest level of independence.
  • Occupational Therapy:
    focuses primarily on working with patients to develop their skills in personal care, cooking, and managing a home, as well as living with assistive devices such as splints.
    Our goal is to help the patient remain independent at home.
  • Speech Therapy:
    provided to improve a patients’ ability with word selection, speech and swallowing.
  • Home Safety Assessment:
    helps the patient and their family learn to live with adaptive equipment and maintain independence in their own home.

Nutritionists

  • Assess and recommend special dietary programs to meet the patient’s needs
