Moorestown Visiting Nurse Association offers home-based behavioral health treatment to patients using a holistic, multi-disciplinary approach. Our visiting behavioral health care staff works with the patient’s primary healthcare provider.

Home care facilitates the patient’s recovery in an environment where family and friends contribute to a sense of security and well-being. The Behavioral Health Home Care Program of Moorestown Visiting Nurse Association provides specialized in-home care for patients and their families who are experiencing medical challenges or who require additional support.

Behavioral health home care reinforces the comprehensive, individualized treatment plan provided by the patient's primary mental health provider. Services focus on the development of personal responsibility and healthy behaviors in the areas of physical, cognitive, affective and social functioning. These services promote independence and self-care in the least restrictive environment, the patient’s home.

Moorestown Visiting Nurse Association’s Behavioral Health Home Care Program is staffed by caring, experienced, certified professionals, and is supervised by a masters-prepared clinician.

Our Behavioral Health Home Care team:

• Communicates an independent assessment of the frequency, intensity and duration of symptoms.
• Identifies health, environmental and behavioral triggers for relapse.
• Intervenes to improve effective coping skills and promote competency in patient and caregiver.
