The Community Counseling Center of Moorestown VNA is no longer accepting new patients or scheduling new appointments. As of March 2022 The Community Counseling Center of Moorestown VNA has closed its licensed outpatient mental health services facility located at 300 Harper Drive, Moorestown, NJ 08057.
How to Obtain Your Clinical Records
Clinical records will be transferred at the request of the client or their new provider. Requests may be made to Moorestown Visiting Nurse Association by calling (856) 552-1300, faxing (856) 552-1303 or mailing an authorization to Moorestown Visiting Nurse Association, 300 Harper Drive, Moorestown, NJ 08057.
Subsequent to closing, clinical records will be transferred to Iron Mountain, 1145 Thomas Bush Highway, Pennsauken Township, NJ 08110, and the custodian of records will be Moorestown Visiting Nurse Association, Medical Records Department, 300 Harper Drive, Moorestown, NJ 08057, (856) 552-1300. Records may be retrieved via a request, with authorization, to such custodian of records.
